Use a hook that is about 2 times smaller than your project hook on your first round.  Ex; If your pattern calls for a size 6 hook, use a size 4 on the first round to pull the yarn through the holes.

Medium - 30x20cm / 3mm thick / 8mm holes
Large - 40x25cm / 3mm thick / 11mm holes
